For Degree Holders Admission

College Of Law
Requirements for Freshmen
The UPHSD College of Law is co-educational and non-sectarian. An applicant for admission must be a holder of a Bachelor of Arts or Science degree from a recognized college or university. The applicant must pass a written admission test and the Dean’s interview.
1. 3 copies of 2x2 colored ID picture and 2 pieces 1x1 colored picture (latest or taken at least 6 months before).
2. Original copy of Good Moral Character Certificate from School Registrar/ Employer/ Brgy. Chairman.
3. Original copy and 2 sets photocopies of Official Transcript of Records with Remarks: “Copy for University of Perpetual Help”.
4. Certified True Copy of College Diploma issued by the School Registrar or Notarized copy of College Diploma.
5. Certified True Copy of High School Diploma issued by the School Registrar or Notarized copy of High School Diploma.
6. Photocopy of Certificate of Live Birth.

Requirements for Transferees
1. 3 copies of 2x2 colored ID picture and 2 pieces 1x1 colored picture (latest or taken at least 6 months before).
2. Original copy of Honorable Dismissal/ Transfer Credential Certificate/ Good Moral Character Certificate issued by the Registrar of the previous school.
3. C-1 (Certificate of Eligibility to Law) issued by the School Registrar of the previous law school.
4. Original copy and 2 sets photocopies of Official Transcript of Records (T.O.R.) issued by the School Registrar of the previous law school.
5. Certified True Copy of pre-law Diploma and Official Transcript of Records (T.O.R.).
6. Photocopy of Certificate of Live Birth.
* The applicant must pass a written admission test and the Dean’s interview.

Graduate School Of Education & Graduate School Of Nursing
Three Sets: 1 for Admission Office, 1 for Office of the Graduate School, 1 for Registrar
1. Original copy and certified true copy of Transcript of Records
2. Good Moral Character Certificate
3. Permit to Study from present employer
4. Two original copies of Recommendation Letters from immediate head/s or past teachers
5. Three copies of latest (not more than 6 months) 2” x 2” colored ID picture
6. Photocopy of Birth Certificate
7. Photocopy of Licensure Examination for Teachers (LET), Nurses (NLE), or other equivalent certificates.
8. Certificate of Employment indicating at least 3 years work experience.
9. Three long brown UPHSD envelop with plastic long envelop to safe-keep documents
10. Fill out application form in three sets
11. Submit all the requirements to the office of the Graduate School

Graduate School Of Business Administration (MBA/DBA)
1. Original copy and certified true copy of Transcript of Records
2. Four copies of latest (not more than 6 months) 1”X1” colored ID picture
3. Letter of recommendation from a former professor
4. Letter of recommendation from employer
5. Certificate of Employment indicating at least 3 years work experience.
6. Fill out the application form
7. Two pieces long brown envelope

Jonelta Foundation School Of Medicine
Freshmen
1. Recommended by the Committee on Admission
2. Bachelor’s degree or its equivalent obtained from a recognized school
3. Complete Transcript of Records
4. Eight (8) passport size, recent photographs
5. NSO authenticated birth Certificate
6. National Medical Admission Test (50 Percentile)
7. Certificate of Good Moral Character from two (2) previous professors.
8. NBI/ Barangay/ Police Clearance
9. Medical & Physical Examination including Chest x-ray ( if accepted)
10. General Weighted Average Ranking
11. 1 long brown envelope

Transferees
1. Honorable dismissal/ certificate of transfer credential
2. Transcript of Records from former medical school

For Foreign Students: (Submit by mail the following documents)
1. Philippine Embassy authenticated Transcript of Records
2. Philippine Embassy authenticated police clearance
3. Philippine Embassy authenticated affidavit of support (notarized)
4. Medical/ Quarantine Clearance
5. NMAT (National Medical Admission Test), Once accepted, a notice of acceptance shall be mailed to the applicant for obtaining a student visa from the Philippine Embassy nearest his/ her place of residence.
