Procedure on ID Processing

  1. Present your official Registration Form to the person in-charge of ID processing.
  2. Fill up properly the form provided by the person in-charge and submit it together with your Official Registration Form.
  3. Wear the prescribe School Uniform.
  4. For male students: observe good grooming (no earrings, shaved beard and mustache and no dyed or colored hair). For female students: No dyed or colored hair.
  5. The person in-charge of ID processing is not liable for any error such as wrong student number, misspelling of the name/surname, wrong address, and contact information reflected on the printed ID of the student. It is the responsibility of the student to check/double check the information encoded by the person in-charge of ID processing before proceeding to ID printing. Otherwise, the student will pay 500 pesos at the Cashier’s Office for the replacement of the ID.
  6. If the printed ID card is damaged upon receiving, the student can ask for a replacement of the ID within the day, free of charge.
  1. Secure an Affidavit of Loss and proceed to the Cashier’s Office to pay for 500 pesos ID fee, then
  2. Follow steps 1 to 6 of procedure on ID processing for New Student ID.

6. Select your student ID from the STUDENT ID NUMBER dropdown list.

7. Select your payment from PAYMENT FOR dropdown list.

8. Enter the AMOUNT you desire to pay

9. Click “SUBMIT”

Scholarship

Pupils ranked No. 1 in each grade level (Grade 1-5) at the end of the school year will be given a 100% discount on Tuition Fee upon enrollment in the next grade level.

JUNIOR BUSINESS HIGH SCHOOL

  1. Entrance Scholarship – Grade school graduates With Highest Honors students will be given a 100% discount on Tuition Fee, graduates With High Honors will be given 75% discount on Tuition Fee only; and graduates With Honors will be given 50% discount on Tuition Fee from a class of not less than forty (40) students upon presentation of a certificate with the school seal Principal. The validity period is for one year only.
  2. Students ranked No.1 in each Year Level (1st to 3rd Year) at the end of the school year will be given a 100% discount on Tuition Fee upon enrollment in the next Year Level.

SENIOR HIGH SCHOOL

  1. Entrance Scholarship – Junior High School graduates With Highest Honors students will be given a 100% discount on Tuition Fee, graduates With High Honors will be given 75% discount on Tuition Fee; and graduates With Honors will be given 50% discount on Tuition Fee from a class of not less than forty (40) students upon presentation of a certificate with the school seal Principal. a. With Highest Honors will be granted 100% discount on Tuition Fee for one (1) semester.

Dean’s List

Students who are Deans list may be given a scholarship grants for one (1) semester if they meet the following criteria:

GWA of 1.25 or better = 100% discount on Tuition Fee

GWA of 1.50 or better = 50% discount on Tuition Fee

Obtain no grade below 88% or 2.00 at any subject

ALTAS Angels (Including NCAA Muse)

Five (5) Scholarships will be given to members of ALTAS ANGELS who meet the following criteria:

  2. Height must not be less thatn 5’5” in height
  3. With impressive vital statistics
  4. Communication skills – conversant in English and Filipino
  5. Good moral character, positive attitude and behavior

Performing Arts Group

Twenty (20) scholarship grants of 25% discount on Tuition Fee will be given to members of the Performing Arts Group who meet one of the following criteria:

  1. Attendance in rehearsals and meetings ad performances
  2. Residency as PAG member of at least 2.5 years
  3. Exemplary talent and skills
  4. Good moral character
  5. Positive attitude and behavior

The Perpetualite / School Paper Staff (All Campuses)

Scholarship grants will be given to selected members of the Editorial Staff who meet the selection criteria stipulated in the Perpetualite Editorial Policies as follows:

a. Editor-in- chief – 100% discount on Tuition Fee
b. Associate Editor – 50% discount on Tuition Fee
c. Managing Editor – 25% discount on Tuition Fee
d. Chief Photographer – 25% discount on Tuition Fee

Supreme Student Council

Scholarship grants will be given to officers of the Student Council as follows

a. President – 100% discount on Tuition Fee
b. Vice President – 100 % discount on Tuition Fee (Las Piñas); 50% discount on Tuition Fee (Molino, Calamba)
c. Secretary, Treasurer, Auditor, Business Manager and PRO – 100% discount on Tuition Fee (Las Piñas); 25% discount on Tuition Fee (Molino, Calamba)

College Student Council (All Campuses)

15% discount on tuition fee will be given to the president of the College Student Council.

Special Scholarships

a. Board of Directors (As per Minute of BOD of UPHSD –Laguna, Aug. 31, 2014)

Each board member is entitled to grant four (4) scholarship grants representing 100% discount on tuition and miscellaneous fee at any given time.

b. Armed Forces of the Philippines

Based on the Memorandum of Agreement between the AFP Educational Benefit System and UPH Medical Center dated June 15, 2005; the following scholarships are to be given:

1. 5 slots – 100% discount on tuition and miscellaneous fees to dependent of military personnel; and

2. 20% discount on tuition fee for active military personnel and their dependents who enroll in any undergraduate courses.

c. Government Sector

For government agencies where the university has direct business dealing for which the latter will benefit from the grant of such scholarship:

1. One (1) slot of 100% discount on tuition fee per government agency at any one time.

2. The applicant must be endorsed to the President by the concerned UPHSD administrator, e.g. HR for DOLE, School Director for Mayor’s Office, etc.

3. To qualify, applicant must meet the following criteria:

a. No failing grade;

b. Must submit character reference/recommendation from head of former school, barangay chairman, and parish priest of head of church where family is a member;

c. To maintain the scholarship, grantee must:

– Carry the regular load every semester

– Must pass all subjects

– Must not shift to another degree program

d. The grant is only for Bachelor’s degree and is not available for board courses, aviation, and TESDA programs.

General Guidelines

  1. A student is eligible for only one scholarship grant at any time.
  2. No combination of scholarship grants of discounts will be allowed.
  3. Scholarship grants for member of specific organizations are non-transferable and unused slots will not be awarded.

  1. For college students, registration form and copy of grades from the previous semester signed by the Registrar.
  2. For Basic Education students, registration form and copy of grades from the previous school year signed by the Basic Education Director.
