Requirements:
- Birth Certificate of the enrolled students
- Photocopy of Registration Form
- 2x2 Picture of the enrolled student
- Photocopy of TOR or Diploma of the Alumni Parent
- Official Copy of Grades (request from the Registrar’s Office)
- Accomplished Anak ng Alumni Discount Form (available at the Alumni Affairs Office)
