LIST OF REQUIREMENTS
GOVERNMENT ACCREDITATION
For International Students
Note: Shall have all documents coming from their country of origin with English translation, if
expressed in another language, and duly authenticated in their country
ORIGINAL COPIES of the following:
1. Authenticated Copy of the Transcript of Records from the school last attended;
2. Certificate of Honorable dismissal from last school attended (if transferee);
3. Passport (bio page, valid visa, latest arrival);
4. Authenticated Certificate of Financial Support from parents or sponsors;
5. Affidavit of consent executed by your parents;
6. Authenticated/notarized copy of Birth Certificate;
7. Certification of good moral character from the former teacher, principal or guidance
counselor (from the school last attended);
8. Police Clearance from your country of origin;
9. 8 pieces of 2×2 pictures;
10. Result of Psychological test from the country of origin; and
11. Medical Certificate from the country of origin
